New patients are currently being accepted. To become a patient, simply follow these easy steps: 

1. Call our office at 480.464.4705 to schedule an appointment

2. On the day of your appointment, bring with you the following items:

1. A completed patient history that you can submit online. Patient History  Form.
2. A valid photo ID.
3. Medical records or information from your previous doctor so medical records can be requested.

3. Walk-in patients welcome, you can complete all necessary documents upon arrival
